Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work.   • Okanogan Behavioral HealthCare has an opening for a Mental Health Case Manager. This position is a school-based position in Tonasket that helps to provide care coordination to clients using a team-based approach and process.

    The primary duties of the position are:

    1) Care Coordination

    a) Connect client to community resources, such as Community Action, DSHS, FYRe

    b) Collaborate with Mental Health Professional, Primary Care Provider, and other agencies involved with client.

    c) Schedule team meetings with agencies involved, to provide wraparound services.

    2) Skill Development

    a) Per the plan in the Electronic Health Record, meet with clients 2-4 times monthly, to provide support with skill development

    3) Documentation

    a) Completed, according to the WAC and agency standards, no more than 5 business days from date of service.

    b) Must be compliant and meet WAC requirements.

    4) Referrals

    a) Update spreadsheet with incoming referrals

    b) Contact referral within 3 days of receiving to schedule enrollment

    c) Check in with referrals, 2-3 times monthly, until an intake has been scheduled.

    5) Enrollments

    a) Ensure the proper consent forms, ROI's and insurance information is gathered and complete for both MHO-A and C&A programs.

    b) Obtain copy of insurance card

    Must possess a high school diploma or GED certificate. Specific knowledge of case management processes and practices are typically learned "on the job." Must have a valid, unrestricted Washington State Driver's License.
