Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work.   • Hours: 40 hours per week. Clinic hours are Monday, Thursday, & Friday,9:00 am to 5:00 pm; Tuesday, 10:00 am to 6:00 pm; and Wednesday, 12:00 pm to 8:00 pm.

    Pay Rate: $20.00 to $23.00 per hour

    Education:High School/GED diploma required with at least one year of experience working with children and adolescents with severe emotional disturbance in a public mental health setting.Bachelor's degree in a mental health related field, such as Psychology, Social Work or Counseling with child and adolescent experience preferred.

    As a Case Manager, you will:

    • Promote healthy development and positive outcomes for families through utilizing case management and rehabilitative strategies with a team of dedicated professionals
    • Assist clients and families to build upon natural support systems and links individuals to other service agencies. Provides additional supportive services to assist with continuity of treatment, linkage to resources, transition from program to program, and transition from program to community resources
    • Connect every client/family to a medical home as well as clinic, school, home, and community-based services
    • Serve as an advocate for the client and family
    • Create and maintain all required documents in the electronic and paper record systems

    Additional Qualifications and Skills

    • Experience working with children and adolescents with severe emotional disturbance in a public mental health setting strongly preferred
    • Ability to make assessments, design treatment plans and document accordingly
    • Proficiency in MS Office applications, specifically Word and Excel
    • Ability to organize and prioritize multiple concurrent assignments and meet productivity goals
    • A valid California driver's license in good standing and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position. In addition, proof of insurance is required prior to driving personal vehicles on company business.
    • Ability to pass criminal/background clearance
