Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. We are seeking a dedicated and compassionate Case Manager to join our team to provide Community Psychiatric Support Treatment (CPST) services to adults, children, and their families. These services include: Individual and Group CPST services, Partial Hospitalization Treatment and the coordination, monitoring and overall service delivery internally and with external systems. Determines intensity of medically necessary services. Some services may be provided off site and in the community. Ensures documentation compliance.
Minimum Requirements: Bachelor’s Degree preferred. A High School Diploma with three years related experience and/or training required. Valid Ohio Driver’s license, use of personal car, and annual training standards as required by the Center.
