Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work. What You Will Do to Change Lives
Under direct supervision, serves as a primary support and case manager for individuals with mental illness and/or co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Shifts Available:
Full Time; DAYS 8:00 am - 4:30 pm; Monday - Friday
Expected starting wage range is $20.53 - $25.07. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

iCare Mobile Engagement Teams (MET) will provide intensive outreach and mobile mental health services to 50 adult individuals with serious mental illness who may be experiencing homelessness and have not engaged in services. Mobile teams will provide outreach, build rapport, develop trust and engage individuals in services including assessment, intensive case management, crisis intervention, housing, medication support, referrals and linkage throughout Sutter and Yuba counties. Teams shall "meet clients where they are” and be responsive and flexible in service delivery and shall utilize various engagement tools such as clothing, food, shoes, blankets, bus passes, hygiene kits, etc. and address barriers to engagement such as transportation, pet care and property storage.
