Case Manager - Mental Health Services jobs in Charlotte, NC
Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work. Anderson Health Services is seeking an innovative Mental Health Therapist who can help youth take ownership of their treatment. We're looking for someone who can provide age- and developmentally- appropriate counseling that keep youth engaged. Caseload is 8-10 youth, ages 5 - 17. The position will provide individual and group therapy sessions to help youth heal from their traumatic experiences.
ESSENTIAL FUNCTIONS
- Provide developmentally appropriate therapy to youth and adolescents.
- Maintain accurate and timely documentation of individualized treatment plans for youth.
- Work collaboratively with a multi-disciplinary team, including Nursing and Case Management.
- Display competence and responsiveness to cultural differences.
MENTAL HEALTH THERAPIST QUALIFICATIONS
Minimum of 1 year experience in adolescent or child mental health.
All eligible candidates must have one of the following licenses in North Carolina.
- LCMHC or LCMHC-A
- LCSW or LCSW-A
- LMFT or LMFT-A
