Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work. Hours: 40 hours per week. Clinic hours are Monday, Thursday, & Friday,9:00 am to 5:00 pm; Tuesday, 10:00 am to 6:00 pm; and Wednesday, 12:00 pm to 8:00 pm.
Pay Rate: $20.00 to $23.00 per hour
Education:High School/GED diploma required with at least one year of experience working with children and adolescents with severe emotional disturbance in a public mental health setting.Bachelor's degree in a mental health related field, such as Psychology, Social Work or Counseling with child and adolescent experience preferred.

Crossroads Family Center is a children/youth outpatient program which provides services to the East region of San Diego County. We provide a range of services including crisis intervention, individual, family, and group therapy, medication support, rehabilitation, case management and drug and alcohol services while incorporating the family systems and wrap-around approach. We provide mental health services to children and youth with Medi-Cal as well as those who are uninsured and underinsured.
