Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Blended Case Management Program
This position is responsible for providing supportive care to identified children with mental health diagnoses and their families in Montgomery and Bucks Counties. This includes helping the family navigate complex systems, gain access to behavioral health services, and coordinating additional resources/supports as identified in the service plan. Responsibilities include service planning, providing case management, and coordination of services with other agency providers to meet the client’s needs along with supportive counseling, with an emphasis on face to face delivery of the service.
