Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Under a contract with the Los Angeles Department of Health Services, a West LA nonprofit community mental health agency has an immediate opening for organized, motivated, and professional candidates for FULL-TIME positions. The ICMS Case Manager will travel to multiple locations in the greater Los Angeles area to meet and work with members to attain and sustain permanent housing in partnership with the Department of Health Services (DHS). The Case Manager’s primary responsibilities are to provide Outreach, Engagement, Linkage, and Housing Location to adults who are homeless, coping with mental health, medical and substance use issues, and in the process of approval for a subsidized housing voucher.

This is a Full-Time position with a 40-hour work week; the agency is currently open for services Monday through Friday 8:30 am -5:00 pm.
Pay: $23.00-$25.00 per hour.
