Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work.     Job Description
    Provide as-needed backup coverage services for the Behavioral Health (BH), Nursing Facility Monitoring (NFM), and OBRA programs including mental health clinical assessments, diagnostic services, treatment planning, linking, monitoring, advocating, consultation, and intervention services.
    Per Integrated Services of Kalamazoo (ISK) guidelines, provides case management services for the BH and NFM programs to mentally ill individuals living in the community (BH - age 55 or over) or in a nursing facility (NFM - all ages) who are at risk of psychiatric hospitalization without on-going support and intervention.
    Per OBRA guidelines, completes comprehensive clinical mental health assessments for individuals diagnosed as having a serious mental illness and/or intellectual/developmental disability who are seeking admission to or are residing in nursing facilities.

    Qualifications
    • The position requires a minimum of a Bachelor’s degree in a human service field plus current state certification or licensure in one of the following Michigan Licensures: RN, LLBSW, LBSW, LLMSW, LMSW, LLPC ,LPC, LLP, LP.
    • A minimum of a bachelor’s degree in a human services field and one year of experience with people with mental illness.
    • Preference will be given to persons with a combination of mental health experience and working with the elderly for at least two years.
