Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work.     Key Responsibilities:

    As a Case Manager, you will play a critical role in guiding individuals on their path to recovery and improved mental well-being. Your responsibilities will include:

    Assessing the needs of clients and developing individualized care plans.

    Connecting clients with appropriate mental health resources and services.

    Monitoring and evaluating progress to ensure effective support.

    Advocating for clients' rights and ensuring access to quality care.

    Qualifications:

    • A deep understanding of mental health issues and treatment options.
    • Strong communication and interpersonal skills.
    • Excellent organizational and problem-solving abilities.
    • The capacity to work both independently and as part of a team.
    • Dedication to upholding high ethical and professional standards.

    Position Requirements:

    • Education: Bachelors in Social Work, Psychology, Human Services or other related fields
    • Experience: 0 or more years of experience in a mental health, social work, or related setting
    • Valid Driver’s License required
