Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Our Pathways to Home team (PATH) program at Aurora Mental Health & Recovery (AMHR) provides intensive community-based services to individuals and families experiencing homelessness. There are 4 primary components to the PATH team. First is Street Outreach which is designed to outreach and engage folks disenfranchised from traditional models of care. Second is Drop In, which is a model of care designed for folks to come as they are, as they want and as they need to get access to a full continuum of services in the mental health and housing sectors. Third is Community Based care where clinicians are providing services in the community to decrease barriers and increase engagement to long term teams. Lastly is our Housing team, which provides the housing case management continuum from assessment to housing case management services to successful completion or maintenance in housing stability. These services are available via referral from any internal and external referral sources. The Case Manager position will work with people experiencing homelessness and will also be working folks that have been matched to our Rapid Rehousing program. This position provides assessments for care coordination, case management, housing support, resources and linkage to appropriate services.
Shift. This position will work fully On-site/In-person, 4x10 hour shifts, days, 8:00 am - 6:30 pm.
Salary for this role. Starting wage is based on experience and company equity. Paid bi-weekly.
Bilingual Language Differential Pay. *rate depends on language requirements for the position and candidate's fluency.
