Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work.     Job Summary: Provides education and connection to counseling and other community resources/services to individuals and groups as needed for substance abuse treatment and recovery.

    Essential Job Duties:

    • Conducts intake screening interviews to determine the need for drug, alcohol, counseling services and/or emergent care services.
    • Assists patients with connecting to community-based services based on the outcome of the screening interview.
    • Utilizes strength-based and trauma-informed approach with patients to support their treatment and recovery.
    • Implements motivational interviewing techniques to engage patients in decision-making process and to develop goals as part of an established treatment plan.
    • Provides case management services to patients with mental health and substance abuse disorders.
    • Collaborates with existing community resources agencies as needed.
    • Monitors patient access and participation in treatment and assists with managing any barriers to treatment.
    • Follows up with patients according to written procedure to ensure timely patient follow-through upon receipt of the referral.
    • Works closely with CCHCI providers to ensure provide ongoing services to patients within CCHCI.
    • Ensures completion of all documentation in EHR according to procedure.
    • Consistently meets service delivery expectations according to the need of CCHCI and according to procedures on Mental Health Substance Abuse Program.
    • Participates in all assigned meetings, staff development, and training as required.
    • Tracks and reports new patients receiving services.

    Required Minimum Qualifications - Education, Experience, Certificates & Licenses:

    • High School diploma/GED and four (4) years of behavioral health experience, OR Associates degree in behavioral health field and two (2) years of behavioral health experience, OR Bachelor's Degree in behavioral health field and one (1) year of behavioral health experience.
    • Current BLS certification.
    • Valid Fingerprint Clearance Card.
    • Driver's License and Proof of Insurance may be required if requesting mileage reimbursement.


    Preferred Qualifications:

    • Case manager experience related to substance abuse and related disorders.


    Required Language Skills:

    • Ability to comprehend and compose instructions, correspondence, and communications in English in both oral and written format.
    • Bilingual in English and Spanish is preferred, but not required.
    • Ability to effectively present information in one-on-one and small group situations to patients, internal providers and staff and other agency staff working in cooperation with the organization.


    Physical Requirements:

    • Ability to occasionally exert enough force to move objects weighing up to 10 pounds.
    • Ability to continuously remain in a stationary position.
    • Ability to constantly move about inside the workplace to assist patients, operate office or medical equipment, etc.
    • Possesses hand-eye coordination and manual dexterity necessary to constantly operate computers, telephone, and other office machinery.
    • Possesses close visual acuity necessary to accurately record and view information on a computer monitor, handwritten and typed documents.
    • Ability to discern the nature of sounds at a normal spoken volume.


    Other Required Knowledge, Skills, and Abilities:

    • Ability to add, subtract, multiply, and divide in all measure, using whole numbers, common fractions, and decimals.
    • Ability to compute rate, ratio, and percent and to draw and interpret graphs.
    • Ability to skillfully gather and analyze data.
    • Ability to perform a variety of assignments and make decisions requiring considerable independent judgment.
    • Displays a high level of critical thinking and problem-solving skills.
    • Knowledge of health plans and community health centers preferred.
    • Computer literacy required.
    • Practical knowledge of assessment tools for substance uses and abuse concerns, such as SBIRT, Audit-C, etc.
    • Knowledge of recovery support systems and 12-step programs preferred.
    • Working knowledge of other drug and alcohol treatment agencies, programs, and resources.
    • Ability to demonstrate sensitivity to cultural diversity and workplace harmony.
    • Ability to prioritize and plan work activities, use time efficiently and develop realistic action plans.


    Work Environment & Conditions:

    • Work environment is typical of a health clinic setting with occasional exposure to communicable diseases, bodily fluids, and hazardous chemicals.
    • Work requires reliable transportation as position requires occasional travel and extended hours to include early mornings, evenings, and weekends.
