Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). Case Manager's years of experience requirement may be unspecified.     POSITION SUMMARY:

    As the Case Manager at All Nations Health Center, you'll play a crucial role in organizing events, supporting clients, and promoting health education. You'll lead outreach programs and help individuals enroll in Medicaid, HMK, and CHIP, directly impacting community well-being. Focused on our Connecting Kids to Coverage program, you’ll assist Native children, families, and expecting individuals in accessing Montana State Medicaid, Healthy Montana Kids (HMK), and Children’s Health Insurance Program (CHIP). Your responsibilities will include coordinating outreach efforts, facilitating local training, and contributing to data analysis and reporting. As a vital team member, you'll educate clients on Medicaid/CHIP/HMK benefits, eligibility criteria, and renewal procedures, while addressing barriers to health and wellness. The Case Manager helps All Nations achieve its vision of a community that is proactive in reaching and maintaining sustainable, optimal wellness.

    DUTIES & RESPONSIBILITIES:

    · Work alongside the Eligibility Specialist to develop and implement programs and events focused on educating individuals about accessing health coverage.

    · Assist in providing preventive care and wellness services, including conducting insurance screenings for adults, children, and pregnant individuals.

    · Maintain precise documentation of activities, enrollment statistics, and client retention metrics following established policies and procedures.

    · Provide education and social support to clients and the community through various outreach initiatives, addressing healthcare concerns.

    · Aid clients in navigating and accessing essential health services and information to fulfill their healthcare needs.

    · Facilitate connections to both internal and external resources for patients and clients seeking assistance.

    · Collaborate with the Finance Department to enhance one-on-one interactions with Medicaid/CHIP/HMK eligible patients.

    · Evaluate the effectiveness of programs and educational materials by conducting comprehensive data collection and analysis.

    · Conduct training programs for fellow health professionals or community health workers as required.

    · Work closely with the Connecting Kids to Coverage team, including internal health informaticists and research evaluators, to assess project progress and ensure grant objectives are achieved.

    · Undertake any additional duties as assigned.

    GENERAL DUTIES:

    · Engage in outreach activities to connect with community residents, aiding them in navigating the healthcare system and providing patient education to enhance care coordination.

    · Act as a valuable member of the interdisciplinary healthcare team, participating in committees as required and contributing to the overall success of All Nations and its employees.

    · Adhere to All Nations Employee Policies and Procedures, as well as specific position and departmental protocols.

    PREREQUISITE QUALIFICATIONS:

    • An Associate’s degree or relevant professional background in fields such as business, communication, community health, prevention sciences, or similar.
    • Basic understanding of insurance and eligibility services in urban, tribal, or federal healthcare settings, or a readiness to acquire this knowledge.
    • Must possess reliable transportation, hold a valid Montana driver’s license, and be open to a criminal background check.

    REQUIRED:

    • Proficient in Microsoft Office applications (Word and Excel), electronic medical records systems, email, and database software.
    • Strong leadership, organizational, written, and verbal communication, and interpersonal skills are vital.
    • Demonstrates sound judgment, decision-making, critical thinking, and problem-solving abilities.

    Maintains confidentiality and privacy consistently, adhering to practice, state, and federal regulations.

    • Shows commitment to developing and practicing excellent internal and external customer service skills.
    • Capable of understanding, following, and enforcing safety procedures.
    • Able to work with a high-risk, minority population while maintaining emotional stability in challenging situations.
    • Effective at working collaboratively in a customer service-oriented team environment.
    • A high school diploma is required.
    • Possesses a valid MT Driver’s License with a clean driving record.
    • Willing to undergo a criminal background check.
    • Employees may be asked to remain or report to work as necessary to fulfill essential tasks for the benefit of the organization. This may involve working non-traditional hours or performing duties outside of regular assignments.

    PHYSICAL DEMANDS:

    Essential job duties may require occasional lifting, pushing, and pulling up to 25 pounds. The ability to occasionally stoop, kneel, crouch and utilize reaching, handling, fingering and feeling movements is required. The ability to hear normal conversational discussion and near and far visual clarity is required.

    Preference in employment is given to qualified Native American candidates, in accordance with Title 42 CFR 36.221.
