Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). Case Manager's years of experience requirement may be unspecified.     Job summaryCoordinate and/or provide services and activities for youth in a group home setting. This includes medical, school and employment appointments and family visitation. Act as a liaison between the agency and other agencies associated with the care of the youth (i.e., DCF, court, school, mental health provider etc.). Collaborates with staff and other key individuals toward implementing a permanency plan.This position will assist the Clinical Director with screening, admissions, and discharge of youth served at Questville Services. This position is responsible for the case management responsibilities associated with the room, board, and watchful oversight of youth in the facility which will include, but not limited to, data entry and implementation of program plans.The incumbent will need to understand basic adolescent development and psychology (particularly at-risk youth and high-risk youth). The incumbent must ensure compliance of state, program and agency regulations associated with providing case management services. Quality care includes: Mental health services, dental, physicals, school, court, recreational, family visits and all related services. This position must be able to function as a positive team player and work in partnership with the other residential staff including liaisons, educational coach, volunteers, and supervisory staff. This incumbent must provide excellent customer service when interacting with both internal and external providers and family members associated with the youth.

    ESSENTIAL FUNCTIONS

    • Carry a client caseload with an emphasis on meeting the environmental and psychosocial needs of the client
    • Provide casework and counseling services to youth and their parents/guardians regarding their program plan.
    • Perform the tasks of a Youth Care Worker when required (See Youth Care Worker Job Description).
    • Arrange and/or supervise visits with parents/guardians and families;
    • Complete data entry of client information into client management system.
    • Consult and review cases with Supervisor and other team members; Perform a number of case management duties including administrative case reviews, preparing reports and participating in facility inspections/surveys.
    • Stay current in job and/or client specific competencies; Actively participate in Continuous Quality Improvement initiatives and activities
    • Other duties as assigned to fulfill the mission of Questville Services; Participate in team meetings, case reviews, staffing, etc.
    • Collaboratively evaluates and plans for treatment of child where problems exist.
    • Cooperates with medical and mental health staff during examination for consultation and staffing of youth
    • Identifies and understands the scope of community resources and how they can meet the needs of youth and families served.

    QUALIFICATION

    Bachelor’s degree in human services, or related field. One to three years of experience required. Passing of all DCF required background check. Must have a perceptive awareness of the needs and responses of youth served. Flexibility and willingness to work evening and/or weekend hours, if needed to carry out all job responsibilities.

    WORKING CONDITIONS:

    • Majority of work will be in a home environment
    • May be exposed to stressful situations
    • Some auto travel required
    • Normal work week will be Monday through Friday with some weekends required
    • Will be required to be on call
