Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). Case Manager's years of experience requirement may be unspecified. Job Summary:
Provide case management services to assigned caseload in compliance with the Bureau of Prisons regulations. Manage and maintain resident files ensuring confidentiality. Assist with ensuring residents' success in programs.
Education and Qualifications:
Bachelor's degree in a social, human services, criminal justice or behavioral science program from an accredited college or university. At a minimum, one year of experience must be working in human services, corrections, or community-based services. Work experience may be substituted for academic studies exchanging one year of work experience in a related field for one year of academic education. Total work experience needed in lieu of the combination of education and work experience is five years. Must have a valid driver's license and social security card issued by the U.S. Social Security Administration, and be a United States citizen, permanent resident, or another person lawfully admitted into the United States. Must pass a federal background check.
