Prime Home Health Position: Registered Nurse (RN) Case Manager/Clinical Supervisor
Reports to: Director of Patient Care Services
Job description: We are looking for dedicated RNs who want to provide quality care to home based patients. You will work one-on-one with the patient and truly impact their life as well as their families. The job is consistent, long-term, and can work with a variety of different schedules. The RN is responsible for providing direct patient care under the supervision of the Director of Nursing. Responsibilities include creating the plan of care, conducting monthly onsite visits, training and orienting LVNs, and working collaboratively with the members of the team to help meet positive patient care outcomes. This is an office-based position. Aside from conducting on-site visits, the RN Clinical Supervisor is expected to work at their office base during regular business hours. The RN Clinical Supervisor will be overseeing LVN patient home care which includes Patient home visits, creating and updating Plans of Care, training and orienting LVNs, and conducting initial patient assessments.
