Case Manager - Alcohol or Drug coordinates the overall plan of care for patients with alcohol or drug problems from admission to discharge. Acts as a liaison between patient/family and service delivery personnel to ensure patients are effectively evaluated and treated. Being a Case Manager - Alcohol or Drug works with management to maintain and improve overall quality of services. Typically requires a bachelor's degree in a related field. Additionally, Case Manager - Alcohol or Drug requires a valid Licensed Chemical Dependency Counselor(LCDC) license. Typically reports to a manager or head of a unit/department. The Case Manager - Alcohol or Drug occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function. Work duties include performing substance use assessments, facilitating group counseling, Moral Recognition Therapy, Cognitive Behavioral Interventions for Substance Abuse and individual counseling in an outpatient setting. Training in Cognitive Behavioral Interventions for Substance Abuse (CBISA) and Moral Recognition Therapy (MRT) is preferred although training can be provided in these areas. Experience and knowledge is preferred in the12 Core Functions of SA Counseling; Individual and group counseling skills; knowledge of potential referral sources; understanding of co-occurring disorders. If there is no experience, the agency will train in these areas. Must have computer skills and the ability to write clearly with attention to detail and be able to plan, prioritize and organize work. Must have the ability to relate to others with compassion, respect and empathy to include staff, clients and referral sources. Certified or Licensed as Addiction Counselor OR Addiction Counselor Trainee (trainee status can be obtainable through the agency). Minimum of 2 years experience in patient care is preferred with required course work in process- or completed. Affirmative Action Employer.
