Case Manager - Alcohol or Drug coordinates the overall plan of care for patients with alcohol or drug problems from admission to discharge. Acts as a liaison between patient/family and service delivery personnel to ensure patients are effectively evaluated and treated. Being a Case Manager - Alcohol or Drug works with management to maintain and improve overall quality of services. Typically requires a bachelor's degree in a related field. Additionally, Case Manager - Alcohol or Drug requires a valid Licensed Chemical Dependency Counselor(LCDC) license. Typically reports to a manager or head of a unit/department. The Case Manager - Alcohol or Drug occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function.     This is a part-time role to support central CT facilities. The primary role of the Drug and Alcohol Recovery Counselor is to coordinate, supervise and facilitate the delivery of Social Work Services, as provided by facility or facility contracted staff, to all residents of the Facility.

     

    Responsibilities: 

    Counsel residents, individually and in group sessions, to assist in overcoming dependencies, adjusting to life, and making changes.

    Collaborate with facility social work, behavioral health, medical, nursing and other departments’ staff, as indicated, in determining the need for and arrangement of substance abuse counseling services for those residents in need of, and agreeable to such services.

    Complete and maintain accurate records and reports regarding the resident's Drug and Alcohol Recovery histories and progress, services provided, and other required information.

    Develop individualized resident treatment plan interventions for Drug and Alcohol Recovery related problems and based clinical experience, and resident histories.

    Review and evaluate residents' progress in relation to measurable goals described in treatment and care plans.

    Modify treatment plans to comply with changes in resident status.

    Provide residents or family members with information about addiction issues and about available services and programs, making appropriate referrals when necessary, and in collaboration with the Inter-disciplinary treatment team.

    Coordinate counseling efforts with mental health professionals and other health professionals such as doctors, nurses, and social workers.

    Monitor trends and patterns of known and reported episodes of substance abuse amongst the facility resident population 

    Attend training sessions in order to increase knowledge and skills.

    Plan and implement follow-up and aftercare programs for residents to be discharged from treatment programs.

    Participate in care conferences and staff meetings, as needed.

    Confer with family members or others close to residents, as needed, in order to keep them informed of treatment planning and progress.

    Instruct other caregivers, as needed, in program methods, procedures, and functions.

    In collaboration with facility administration and the staff development coordinator, provide or arrange as needed staff training regarding substance abuse.

     

    Qualifications:

     

    EDUCATIONAL REQUIREMENTS

    Master’s Degree in Social Work (MSW) from a CSWE accredited graduate degree program is highly preferred.

     

    REQUIRED CREDENTIAL(S)

    CT Certified Substance Abuse Counseling Certificate (CT DPH)

     

    EXPERIENCE:

    Minimum of 2 years of having provided substance abuse counseling.

    Must have working knowledge (experience preferred) with various software programs such as WORD, EXCEL.  Must be able to manage multiple projects and work with all disciplines in a challenging multi-cultural environment.  Must be knowledgeable of Quality Assurance/Improvement process as well as state/federal regulations and surveys.  Must be able to speak and write the English language.
