Case Management Director oversees a staff of case managers responsible for patient care coordination. Develops and implements case management programs, including utilization review, intake or discharge planning, and managed care contracting or negotiation. Being a Case Management Director evaluates patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Seeks treatments that balance clinical and financial concerns with the family's needs and the patient's quality of life. Additionally, Case Management Director contributes to the development and improvement of clinical care pathways that enhance cost effectiveness while providing quality care. Typically requires a bachelor's degree. Typically reports to top management. May require Registered Nurse (RN). The Case Management Director typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget.     Responsibilities:

    • Lead the Case Management department by developing and implementing strategic plans, managing resources effectively, and fostering a culture of professional development for your team.
    • Champion best practices in case management, care coordination, and discharge planning, ensuring a smooth and efficient patient journey.
    • Oversee daily operations, including staff management, technology utilization, and workflow optimization, to achieve the highest standards of quality and efficiency.
    • Leverage data analytics to identify areas for improvement and implement sustainable solutions that deliver positive patient experiences.
    • Build strong relationships with key stakeholders across the hospital, including physicians, nurses, social workers, and external agencies, to ensure a cohesive approach to patient care.
    • Ensure compliance with all regulatory requirements and hospital policies.

    Qualifications:

    • Bachelor's degree required, Master's in Nursing, Health Administration, or Business Administration a plus.
    • Active RN license in good standing or LCSW license required.
    • 5 years of clinical experience with a strong foundation in patient care.
    • 3 years of experience in acute care hospital case management.
    • 2 years of prior leadership experience in a case management setting.
    • Strong leadership, communication, and interpersonal skills.
    • Ability to manage multiple priorities, work independently, and thrive in a fast-paced environment.
