Case Management Director oversees a staff of case managers responsible for patient care coordination. Develops and implements case management programs, including utilization review, intake or discharge planning, and managed care contracting or negotiation. Being a Case Management Director evaluates patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Seeks treatments that balance clinical and financial concerns with the family's needs and the patient's quality of life. Additionally, Case Management Director contributes to the development and improvement of clinical care pathways that enhance cost effectiveness while providing quality care. Typically requires a bachelor's degree. Typically reports to top management. May require Registered Nurse (RN). The Case Management Director typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. Capable of resolving escalated issues arising from operations and requiring coordination with other departments. To be a Case Management Director typically requires 3+ years of managerial experience.   • Note: This hourly, non-exempt role offers a starting compensation of $17.55 to $20.20 per hour, based on case management experience, with potential for increases as caseload and expertise grow.

    HealthKeeperz seeks an individual who is passionate about serving the community members of Mecklenburg County and surrounding counties. The HealthKeeperz Community Alternative Program (CAP) Case Manager ensures qualified Medicaid clients are safe at home and receive all the products and services needed to stay home. HealthKeeperz CAP Case Managers work to match clients with resources in their community. The education credentials for the CAP case manager position are as follows.

    1. Bachelor’s degree in social work from an accredited school of social work and one (1) year of directly related community experience (preferably case management) in the health or medical field directly related to home care, long-term care, or personal care and the completion of an NC Medicaid certified training program within 90 calendar days of employment; OR

    2. Bachelor’s degree in a human services or equivalent field from an accredited college or university with two or more years of community experience (preferably case management) in the health or medical field directly related to home care, long-term care, or personal care and the completion of an NC Medicaid certified training program within 90 calendar days; OR

    3. Bachelor’s degree in a non-human services field from an accredited college or university with two or more years of community experience (preferably case management) in the health or medical field directly related to home care, long-term care, or personal care and the completion of an NC Medicaid certified training program within 90 calendar days; OR

    4. Nurse who holds a current North Carolina license with two (2) year or four (4) year degree and one (1) year case management in home care, long-term care, personal care or related work experience and the completion of an NC Medicaid-certified training program within 90 calendar days; OR

    5. An individual with a bachelor’s degree or who holds a nursing license as described above, without the number of years of experience, may be designated as an apprentice or a trainee and shall be hired to act in the role of case manager. The supervisor of the case manager shall provide direct supervision and approve all waiver workflow documentation and tasks.
