Position Summary:
POSITION SUMMARY: The Director Case Manager position is directly responsible for overseeing and administering all case management functions of the organization. This position is a member of the organization's leadership team and reports to the Chief Nursing Officer. The Director of Case Management Services develops, drafts, and reviews organization policy and prepares, reviews, and approves organization reports, assures deliverables of grants are met and manages assigned grants. This position represents the organization at meetings with outside entities and members of the public. Plans assigns and supervises the work of the organization's case management staff and associated support staff. Maintains availability to ensure optimal supervision and action regarding supervision and oversight of assigned case management duties. Works under minimal supervision with extensive latitude for the use of initiative and independent judgment.
