    Position Summary:

    POSITION SUMMARY: The Director Case Manager position is directly responsible for overseeing and administering all case management functions of the organization. This position is a member of the organization's leadership team and reports to the Chief Nursing Officer. The Director of Case Management Services develops, drafts, and reviews organization policy and prepares, reviews, and approves organization reports, assures deliverables of grants are met and manages assigned grants. This position represents the organization at meetings with outside entities and members of the public. Plans assigns and supervises the work of the organization's case management staff and associated support staff. Maintains availability to ensure optimal supervision and action regarding supervision and oversight of assigned case management duties. Works under minimal supervision with extensive latitude for the use of initiative and independent judgment. This position has responsibility for demonstrating the core values of CareSTL Health: Customer Service, Community, Commitment, Compassion, and Competence.

    ESSENTIAL FUNCTIONS:

    The following information is considered the definition of essential functions but does not restrict the tasks that may be assigned. The Director of Case Manager may be reassigned duties and responsibilities at any time due to reasonable accommodation or other staffing reasons.

    • Directs and coordinates the daily operations of all case management staff assigned to provide services to all patients at the various clinical sites.
    • Develops goals for case management staff consistent with the agency's strategic plan. Oversees and develops comprehensive audit reports to evaluate staff compliance with goals, policies, and procedures. Monitors multiple databases to ensure accurate employee data entry. Prepares Completes and maintains reports of all required documentation about clients as well as reviews and maintenance of medical records as needed.
    • Participates in the agency's strategic planning process through the identification of short- and long-term goals. Develops, drafts, reviews, and approves agency policy and procedures related to the organization and treatment consistent with agency goals. Implements policy and procedure changes to include training of staff and vendors.
    • Screen and assess clients for their appropriateness for organization services and provide referrals for identified needs of the client and crisis intervention when needed and provide case management to coordinate other services and resources to specialized maternal child case management services to clients and their families with needed
    • Participate in the Maternal Infant Health Coalition and other relevant local community meetings and engage in collective impact and community engagement activities to improve organizational outcomes while developing and maintaining strategic working relationships with key community partners related to maternal and child health priorities.

    • Ability to write clear, concise quarterly and bi-annual project reports for the organization, and to compose educational materials in response to provider needs assessments
    • understanding of issues such as patients facing or at high risk for HIV, addiction, mental health challenges, and other social determinants of health and a comprehensive knowledge of pediatric and maternal prevention, testing, and care guidelines with the ability to assess community needs related to maternal and child health and facilitate individual or group orientation or educational
    • Assist management in program evaluation through data analysis, reports, and recommendations and assist in the implementation and development of evidence-based or promising practice programming around infant mortality.
    • Understanding of factors contributing to health care policy changes and quality assurance/improvement efforts sessions (as required) while maintaining an effective working relationship with other area service providers and ability to work with various staff levels, including supervision of student interns
    • Familiarity with processes for health care quality improvement and systems change, various insurance plans and deliverables, and knowledge and practice of confidentiality and issues of diversity
    • Ability to prioritize and manage multiple tasks, and work independently as well as part of a team

    JOB REQUIREMENTS AND QUALIFICATIONS:

    Education:

    • Graduated from an accredited School of Nursing and licensed as a Registered Professional Nurse in the state of Missouri
    • Current CPR Certificate
    • BSN required
    • Certification in Case Management highly preferred
    • MSN preferred

    Experience:

    • 2 years of experience as a Case Manager in a community health center and/or hospital setting

    Knowledge, Skills, and Abilities:

    • Knowledge of information and techniques for providing excellent patient care
    • Knowledge of administrative and office procedures and systems such as electronic transmission via facsimile and/or internet
    • Good computer skills including the use of office procedures and terminology
    • Good self-motivational and organizational skills
    • Ability to maintain confidentiality
    • Ability to work all health center sites
    • Understanding of the complexities of integrating services
    • Knowledge of data management through use of IT

    Transportation Requirement:

    • Must have reliable transportation

    • Copy of Diploma or Official Transcripts
    • Current Registration with the Missouri Family Care Safety Registry
    • Annual PPD or Chest X-ray
    • Copy Nursing and licensed as a Registered Professional Nurse
    • Copy of CPR Certification

    Training Requirements (licenses, programs, or certificates)

    • Current professional registration with the State of Missouri as a Licenses/Registered Nurse
