JOB TITLE: Social Services Planner
REPORTS TO: Case Manager Supervisor
GENERAL SUMMARY:
Under the supervision of the Case Manager Supervisor, responsible for all aspects of the ADRC including research, development, implementation, monitoring outcomes, and preparing reports. Responsible for case management and coordination of the National Family Caregiver Support Program and KY Caregiver Program.
ESSENTIAL DUTIES AND TASKS:
· Answer the phones and complete a pre-screening/intake to connect the consumer to appropriate services/agencies
· Facilitate program enrollment for publicly funded services
· Assist all callers who request information through referrals, information, and/or screenings
· Input all calls into SAMS at the time of contact and make sure all critical report data is complete
· If the consumer is referred to the Homecare program or Title III Meal Program, add the consumer to the waitlist within SAMS and notify the Case Manager Supervisor of placement for internal tracking purposes
· Check SAMS call summary report for missing data once weekly, at minimum bi-weekly.
· Attend job development training and staff meetings
· Respond to calls from the public and provider agencies concerning programs, data, and resources
· Identify service needs, resources, and barriers as they relate to older adults and persons with disabilities
· Complete online Medicaid Waiver applications for consumers applying for the Home and Community Based Waiver Program
· Participate in and assist with the coordination of support groups, health fairs, and outreach events
· Coordinate the National Family Caregiver Support Programs and Kentucky Caregiver (Grandparent) Programs.
· Understand the Older Americans Act, state policies, and regulations.
· Prepare financial reports, monthly billing, and other reports as required (voucher processing)
· Preforms in-home assessments/visits as necessary
· Maintain a computerized client tracking system
· Perform other duties as assigned
WORKING CONDITIONS:
Most work is performed in an office, but some are performed throughout the ADD service region. Some overnight travel may be required for training purposes. This includes traveling under potentially adverse weather conditions and into rural areas to visit client homes.
EFFORT:
Must be able to walk, see, hear, and speak. Intermittent sitting, standing, stooping, fingering, sorting and reaching. Occasional light lifting which can be accommodated. Work requires a high level of mobility.
MACHINES, TOOLS & EQUIPMENT:
Must have a valid driver's license. Typical use of computers, scanners, printers, calculators, copiers, shredders, etc. as well as an automobile.
MINIMUM QUALIFICATIONS:
High school diploma or equivalent and preferred three years of direct experience working with the elderly or disabled. Reasonable accommodations may be made to those who are able to perform the essential duties of the job. Must be able to pass any required drug test and new hire physical exam.
