Job Description:

Social Worker is responsible for managing all the daily functions of Social Services Department. Work with participants, family members and other departments to identify necessary programs and services. The duties include comprehensive assessments, document notes in the participant records and meet the regulations and requirements.

Responsibilities for Social Worker

· Train the staff in the social work department.

· Staff scheduling, approve staff's timesheet, time off/vacation requests, and assist in human resources processes as needed.

· Perform review for department staff.

· Provide service skills and emotional support to prevent staff overwhelming.

· Complete care plan, mental assessment and evaluation with score and signature simultaneously with 3-month schedule and 6-month schedule.

· Ventilate client negative feeling as needed.

· Provide the psychosocial support as needed.

· Prevent deterioration of mental conditions and social isolation.

· Help new participants admission either transfer based on freedom of choice or new application.

· Conduct discharge plan, process, and referral options.

· Connect with county and state social services departments to update the social services policy

· Coordinate with the nurse, activity, or other department to reach a specific goal or mission.

· Coordinate weekly department meeting and participate weekly managers meeting

· Participate in marketing activities

· Perform other duties as assigned.

Qualifications

· Valid LBSW or LMSW required

· Strong critical and strategic thinking, analytical and planning skills

· Pass the Criminal background check

· Ability to juggle multiple tasks at once

· Excellent interpersonal skills

· Ability to give constructive criticism to employees

· Ability to create policies for employees to follow

· Amazing leadership abilities

· Strong English communication skills, bilingual preferred

· Have knowledge of state and county compliance regulations

Job Type: Full-time

Pay: $20.00 - $30.00 per hour
