Makes day-to-day decisions within or for a group/small department. Has some authority for personnel actions.     Position Summary

    The Breast Center Navigator is responsible for the care coordination and implementation of best practice elements across the health care continuum. Works in partnership with patients, care givers, physicians, physician practices, hospital personnel, and post-acute providers to collaborate, educate and implement strategies to improve the care of Breast Center patients.

    Essential Functions
    • Implements best practice elements related to breast health across the health care continuum to ensure high quality coordinated care of patients.
    • Reviews patients regularly and evaluates patient progress with plan of care. Communicates plan of care to patient and family and solicits concerns, questions, and issues for resolution.
    • Address patient phone calls regarding clinical concerns. Follow up on imaging results and communication of results to patients.
    • Schedule surgeries, and obtain prior authorizations for surgeries.
    • Assist physicians with Tumor Board preparation.
    • Coordinate patient care and throughput from initial visit, surgical intervention, follow-up care and post-procedure treatment.
    • Perform patient call back for imaging and pathology reports.
    • Assist in Breast Clinic rooming patients and maintaining an efficient flow from assessment through discharge.
    • Assist physician with in-office procedures.
    • Facilitate imaging, biopsies, and surgical procedures of the Breast Health patient per the physician’s plan of care.
    • Provide education to patients with benign breast disease.
    • Supports the breast cancer patient and their family in understanding and coping with the strong emotions that accompany a breast cancer diagnosis. Remains the constant caregiver over the continuum of treatment through survivorship, overseeing the patient and family’s emotional and educational needs and providing resources and referrals.
    • Assist with breast biopsy’s in the Radiology department.
    • Work as an interdisciplinary care team member to manage the care journey and anticipate care support and education needs of patients diagnosed with breast disease or breast cancer.
    • Working with the hospital nurse navigator, TCC and MSW perform transition of care management to CHF breast patients after discharge from the hospital, successfully bridging the transition from hospital to outpatient care and preventing readmissions.
    • Remotely manage the care of breast patients, triage calls, and escalate high risk patients using remote assessment skills and nursing expertise with physician oversite.
    • Works closely with surgeons to facilitate physical assessments, medication and history reconciliation, in depth disease management education and life style support, medication administration and identifying gaps in social determinants of health of patients.
    • Exhibit an in-depth knowledge of disease processes, pharmacology, and current guidelines to develop comprehensive care support for patients and promote successful outcomes.
    • Exhibits strong communication skills using empathy and social intelligence to evaluate patient needs and promote patient engagement and self-care.
    • Community resource referral, patient education, and navigation at the entry point and across the continuum of care.
    • Actively participates in the process for operational improvement and quality improvement.
    • Involved in learning and self-improvement.
    • Provides leadership through communication, education and management.
    • Provides advocacy for the patient and family.
    • Initiates and maintains positive relationships with coworkers and physicians.
    • Demonstrates commitment to the mission, vision and values of UH.
