Position Summary:
The Patient Navigator is the key position at the core of the Sequoia Breast & Diagnostic Center located at 2900 Whipple Ave. The Center offers a personalized, individualized approach to screening and diagnosis for breast health. The Patient Navigator provides: Direct patient advice, guidance, education, referral and follow-up for breast center patients; serves as the patient's personal health assistant and is the primary contact for questions, coordination of care, and navigation through a cancer diagnosis; oversees quality measurements, continuous quality improvement and all patient satisfaction initiatives on topics of Breast Health and Women's Imaging; outreach to community groups, partners, and physicians on issues of breast health; program development and long term planning, working with marketing and strategy as needed; oversight of center connectivity to the full hub services as part of Women's Integrated Health at Sequoia; and possible other duties include the clinical assistant to the
mammographer. The Patient Navigator is expected to work closely with Center staff and Center physicians to create seamless coordination of care, and be a key participant in service line development. Over time, this position is expected to become patient navigator lead for Women's Integrated Health at Sequoia, with a shift of focus to the full range of women's services and supervising a second position specializing in breast services. Incumbents in this position shall comply with all applicable laws and rules including the Dignity Health Standards of Conduct and Dignity Health policies and procedures that apply to his/her job responsibilities, seek guidance when in doubt, promptly report any potential or suspected violation of Dignity Health Standards of Conduct, Dignity Health policy or procedure or applicable laws/regulations and when requested, assist Dignity Health personnel in investigating all allegations of violation. When called upon, the incumbent shall assist in risk evaluation and work collaboratively with facility/entity leaders to implement, monitor and mitigate risk corrective plans outlined by the Compliance program.
