    1.0, Shift: Day, Schedule: M-F 8-4:30pm, rotating leader call

    Hybrid position with 2 days per week in office and 3 days per week working remotely.

    Needs current driver's license, cell phone and vehicle to use for work.

    GENERAL DESCRIPTION

    The Home Health Clinical/Nurse Manager oversees employees to assure coordinated, interdisciplinary care to patients in their home setting. Participates in development of department policies, procedures, implements quality management plan and evaluates for compliance. Responsible for clinical standards including targeted QAPI outcomes. The Manager performs all duties in a manner that promotes team concepts and reflects MultiCare’s Mission, Vision and Values.

    PRINCIPAL ACCOUNTABILITIES

    • Actively supports and incorporates the Mission and Core Values into daily activities. Treats all others with respect and demonstrates excellence, justice and compassion in daily work and relationships with others.
    • Demonstrates services excellence and positive interpersonal relations in dealing with others, including patients, families. Caregivers, employees, all leadership, medical staff, volunteers and community members to assure positive relations are maximized.
    • Develops individual objectives consistent with the MultiCare Health System’s Patient Satisfaction, Outcomes and Publicly reported measures.
    • Counsels and provides performance management to employees. Holds staff accountable for patient outcomes.
    • Facilitates communication, coordination and problem solving with physicians, patients, families, staff and payors. Serves as a liaison to hospital, acute care managers and community agencies.
    • Participates with the Director, other Home Care Leaders, Quality Assurance RNs, Outcomes Specialist and others in developing, evaluating and revising home care policies, procedures, standards, competencies, quality and risk management and objectives to assure they cover clinical operations and processes to meet industry standards and regulatory requirements.
    • Supports coordination of admission between MYMH Care Coordination, Home Health Intake and Home Health Scheduler by daily oversight of staffing functions to provide service to those in our community.
    • Oversees quality of documentation to assure all regulatory and quality measures are met.
    • Age related competency for Pediatric, Adolescent, Adult and Geriatric patients.
    • Oversees staff providing direction and guidance and administering management functions within the provisions of MultiCare policies and standards and federal, state and local regulations.
    • As needed provides age-appropriate individualized assessment, planning, implementation, and evaluation of health care for patients from a diverse range of cultural backgrounds, and which reflects the philosophy, goals and objectives of nursing services and YVMH.
    • Able to work independently in home settings; nursing home settings; assisted living settings coordinating care with caregivers and family members.
    • Able to use teach back skills and other tools to engage the patient and family in goal setting and self-management.
    • Able to complete in Home Health the OASIS assessment tool based on the individual patient; become OASIS certified within 1 year of hire.
    • Superuser for clinical team with ability to documents on laptops at point of care as outlined in policies/procedures


    Bilingual skills (English/Spanish) preferred

    Requirements

    • Bachelor's degree in Nursing, or obtained within three (3) years from the initial date of hire; RN with 25 years of service may be considered in lieu of education agreement
    • Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred
    • Washington State Registered Nurse license (RN)
    • Minimum five (5) years of clinical experience in professional or technical role/function preferred
    • Minimum three (3) years of experience in a supervisory or lead capacity preferred

    Pay and Benefit Expectations

    We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $88,842.00 - $133,263.10 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
