Bereavement Services Director - Home Care jobs in Nevada
Bereavement Services Director - Home Care designs, plans and directs the home care bereavement program. Trains and coordinates a staff of bereavement counselors. Being a Bereavement Services Director - Home Care assigns counselors to families as necessary, and ensures services fulfill the needs of the family. Requires a bachelor's degree. Duties:
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed
- Monitor and evaluate patient progress and adjust care plans as needed
- Educate patients and their families on disease management, medication administration, and self-care techniques
- Coordinate with other healthcare professionals to ensure continuity of care
- Perform Durable Medical Equipment (DME) setup and maintenance
- Collect and process specimens for laboratory testing
- Provide health coaching and support to patients and their familiesQualifications:
- Valid Registered Nurse (RN) license in the state of practice
- Minimum of 1 year of nursing experience, preferably in home health or a related field
- Proficiency in electronic medical record systems, such as Note-E-Fied, DataSoftLogic, etc.
- Strong assessment, communication, and documentation skills
