    Lumina Hospice and Palliative Care founded as Benton Hospice Service in 1980, is a community-grown nonprofit, that provides compassionate care and support for individuals, families and caregivers facing serious illness or end of life.

    Lumina Hospice & Palliative Care is seeking a part-time (.8 FTE) Bereavement Coordinator to join our team. As a Bereavement Coordinator you will develop, implement, and supervise the comprehensive Lumina Hospice Service and Palliative Care bereavement program. Our goal at Lumina Hospice & Palliative Care is to maximize the comfort and health of patients and families consistent with hospice philosophy and the organization’s policies and procedures.     Your responsibilities will be to:

    • Maintains standards of excellence in accordance with principles set forth by the profession of the Bereavement Coordinator and the Guidelines for Bereavement Care of the National Hospice and Palliative Care Organization.
    • Functions as a member of the interdisciplinary group (IDG) to formulate a plan of care for patient/family and/or other caregivers in consultation with other IDG members.
    • Develops and implements an appropriate bereavement plan for each hospice family.
    • Plans, implements, and evaluates bereavement support groups and social events.
    • Develops, implements, and evaluates educational programs for families, staff, and the community on loss, grief, and related topics.
    • Provides individual and family counseling and referrals for hospice family members and the public, as appropriate.
    • Trains and supervises bereavement volunteers; participates in evaluation of bereavement volunteers.
    • Participates in the orientation, training and support of hospice staff, as appropriate.
    • Maintains overall responsibility for patients’ bereavement records in accordance with Lumina’s record retention policies.
    • Documents all relevant family contacts and interventions.
    • Oversees Lumina’s bereavement resources.
    • Participates in IDT meetings and other staff meetings as appropriate.
    • Contributes to communication, positive work environment, and team effort.
    • Responsible to maintain knowledge and skills and to effectively complete assigned duties including knowledge of organizational policies and information presented at agency meetings and in-services.
    • Serves as assigned on agency-wide initiatives and/or activities.
    • Participates in organization-wide initiatives and activities, including contributing to QAPI programs and updates.
    • Understands and adheres to all policies including Privacy Policy; Vulnerable Persons Policy; and all policies and procedures contained in the Employee Handbook.
    • Completes documentation at the time of visit or as soon as possible following the visit. All documentation must be completed within 24 hours.
    • Performs other duties as assigned.

    Qualifications:

    • Commitment to serving the needs of the Bereaved.
    • Master’s degree from an accredited school in social work, counseling, or related program and three years of post-Master’s experience in a social service setting, preferably dealing with grief and loss issues, or equivalent combination of education and experience LCSW, LMFT, LPC licensures preferred.
    • Or a Master’s in Divinity with Board Certified by National Association of Catholic Chaplains, or Association of Professional Chaplains, the National Association of Jewish Chaplains, or Spiritual Care Association/Healthcare Chaplaincy Association or Association of Clinical Pastoral Education is required.
    • Demonstrated knowledge of grief and loss theory and evidence-based interventions.
    • Experience in individual, family, and group counseling and with children, teens, and adults, particularly in the areas of terminal illness, loss, and grief.
    • Experience in program development and outreach.
    • Ability to work as a member of an interdisciplinary team.
    • Requires strong written, oral and interpersonal communication skills.
    • Must be reliable, flexible and have the ability to work independently and as a member of a team in delivering assistance within job requirements.
    • Intermediate skills in email, word processing, internet, keyboarding, and the ability to learn to use medical software to complete accurate and timely charting is required.
    • Bi-lingual, Spanish speaking ability preferred.
    • Ability to work as a member of an interdisciplinary team.
    • Must have a valid driver’s license and acceptable driving record and have access to a reliable automobile to travel to and from patient/clients’ homes as well as facilities and events.
    • Experience working with minimal supervision while delivering effective and on-time results.
    • Strong organization skills, time management, and attention to detail.
    • Basic life support CPR card.

    Schedule: Hours fall within the 8-5 Monday through Friday work week, but time is also required during evenings and weekends to meet family and patient needs.

    • Preferred applicant will be asked to pass a background check and drug screen following a job offer.
