This full-time or part-time position will work under the supervision of an RN, PT, or ST assists with activities of daily living, homemaker skills, and delegated plan of care to meet the needs of the patient in the home setting. This may include personal care, housekeeping tasks, and delegated exercises.
Other responsibilities include:
- Observe and report physical and/or behavioral changes in the client to the Home Connections office, Home Health/Hospice nurse, or Department of Social Services caseworker.
- Document patient/client care provided in a timely and accurate manner.
- Assist in maintaining a clean and safe patient/client environment.
- Follow the assigned schedule and informs clients if the scheduled appointment to the patient/client's home needs to be modified.
- Complete the annual aide competency training/testing and participates in the ongoing aide training.
- Provide the Home Connections office with an accurate and timely record of mileage and other required employee-centered documentation.

Qualifications
Graduate from high school or in possession of a GED certificate.
Completed or is eligible for the nursing assistant certification program. Maintain certification by attending required hours of in-service training.
Previous experience desired.
Must have or be eligible for South Dakota’s Drivers License.
