We are seeking a highly motivated and experienced Case Manager to join our team in Denver, CO. The successful candidate will be responsible for providing case management services to clients, including assessment, planning, implementation, coordination, monitoring, and evaluation of services.

Responsibilities:

• Conduct initial assessments of clients to determine their needs and develop individualized service plans
• Coordinate and monitor the delivery of services to clients, including medical, social, educational, and vocational services
• Provide ongoing support and advocacy to clients to ensure their needs are met
• Collaborate with other service providers and community resources to ensure clients receive comprehensive services
• Maintain accurate and up-to-date client records and documentation
• Participate in case conferences and team meetings to ensure effective service delivery
• Provide crisis intervention and support to clients as needed

Requirements:

• Bachelor's degree in social work, psychology, or related field
• Minimum of 2 years of experience in case management
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of community resources and services
• Ability to maintain confidentiality and adhere to ethical standards
• Valid driver's license and reliable transportation
