    Primary Purpose :

    In partnership with the greater Fairfield County Coordinated Access Network (CAN), New Reach provides supportive housing and rapid-rehousing case management services to assist individuals and families with exiting homelessness into permanent housing through the provision of housing supports and strategies.

    The services are provided primarily to low-income individuals and families experiencing homelessness in the greater Danbury area.

    The housing caseworker acts as the first point of contact to provide support and advocacy, housing counseling, and community linkages with the ultimate goal of housing stabilization.

    Essential Functions :

    • Provide comprehensive home and community-based case management services, with a focus on housing stabilization supports.
    • Coordinate intakes with households referred by the Coordinated Access Network (CAN) and Shelter Plus Care program in the greater Danbury area.
    • Provide education to clients and property owners / managers about the program's services to engage all parties in appropriate service components.
    • Co-create housing stabilization goal plans with clients and other parties involved in services to :
    • Assist clients with preparing for housing, including affordable housing search, transition to housing, tenancy skills building, fiscal preparedness for independent rent payment, and housing maintenance.

    Refer and support engagement in appropriate services to meet the household's needs.

    • Work collaboratively and maintain positive, professional communication with property owners / managers, subsidy administrators, and community providers to support housing acquisition, retention, and overall stability.
    • Develop a strong community presence to educate others about the program, advocate for housing rights, and mediate housing concerns effectively.
    • Actively participate in agency and community-based meetings and training to engage in programmatic, operational, and client-service best practices
    • Maintain timely and accurate documentation for all client files, including assessments, goal plans, client services, referral outcomes, housing inspections, rental assistance, and case notes.

    Comply with the agency, funder, and statewide HUD HMIS standards.

    Assist in program development and quality assurance activities as directed.

    Qualifications :

    • BA / BS in Human Services or related field, OR associate degree and two years of human services experience required.
    • Highly motivated, with excellent interpersonal, oral, and written communication skills.
    • Ability to provide intensive and comprehensive services to households experiencing homelessness, including advocacy and mediation skills.
    • Ability to work independently and as part of a team to support the mission of New Reach.
    • Proficiency in Microsoft Outlook, Word, Excel, and PowerPoint.
    • Valid driver's license and access to reliable transportation with proof of insurance.
    • High level of organizational skills to prioritize and manage multiple projects simultaneously.

    Preferred Qualifications :

    • Experience with housing, housing law, and provision of services to the identified population preferred.
    • Experience working with individuals affected by mental health and or substance use.
    • Bilingual in Spanish, both written and oral
