This role presents an exciting opportunity to contribute to our client's renowned PACE program, designed to empower seniors to stay in their homes and communities through comprehensive medical care and community-based services.

    Responsibilities:

    • Oversee the efficient operation of our client's high-performing clinic, ensuring clear workflows and adherence to protocols.
    • Lead, coach, and develop the clinic team, including RN Case Managers, Clinic RNs, and Medical Assistants, fostering engagement and well-being.
    • Analyze data trends to identify opportunities for improvement, providing recommendations to enhance clinic operations.
    • Ensure proper documentation of interventions as per participants' care plans and promptly respond to physician orders.
    • Evaluate the quality and effectiveness of coordination and services, collaborating with medical practice leadership.
    • Serve as the audit lead for clinical and nursing elements during CMS/DHCS audit reviews, working with the Quality Improvement Director.
    • Develop and monitor corrective action plans to ensure ongoing compliance.

    Requirements:

    • Associate or Bachelor’s degree in nursing required.
    • Unencumbered registered nurse (RN) state license required.
    • Minimum of two (2) years of experience overseeing an ambulatory clinic operation.
    • Minimum of two (2) years of experience in a clinical setting with a frail or elderly population.
