    Our Award Winning Client is actively seeking a Licensed Nurse Practitioner to spearhead their innovative team, dedicated to reshaping the care landscape for individuals affected by Serious Mental Illness (SMI) across the nation. This role requires an individual capable of leading medical teams and collaborating in a high-touch care model aimed at significantly enhancing health outcomes and promoting independence among SMI populations.

    Primary Responsibilities:

    • Lead the medical component of team-based care, imparting clinical knowledge to Guides and Community Resource Guides.
    • Carry out comprehensive clinical assessments, capturing unaddressed conditions and coordinating necessary additional studies.
    • Develop personalized care recommendations and liaise with both internal programs and external providers to ensure continuity of care.
    • Support care coordination, ensuring effective communication across a multidisciplinary team to manage individual health outcomes effectively.
    • Drive initiatives in medication reconciliation and maintain robust connections with external health care facilities including CMHCs, FQHCs, and hospitals.

    Required Qualifications:

    • Master’s degree or higher in Nursing.
    • Active Nurse Practitioner license in Washington State.
    • Minimum of 5 years of experience in clinical settings, preferably within primary care or similar environments.
    • Proven expertise in managing populations with complex health needs, including behavioral health or Substance Use Disorders (SUD).
    • Strong ability to engage in home visits and adapt to varied work environments.

    Technical and Professional Skills:

    • Proficient in comprehensive health assessments and gap analysis in care.
    • Skilled in using Microsoft Office Suite and capable of handling electronic health records.
    • Knowledgeable in local healthcare networks and community-based resources.
    • Experienced in HCC Coding is preferred.

    Desirable Attributes:

    • Demonstrates empathy, approachability, and the ability to communicate effectively.
    • Culturally sensitive and aware of social determinants impacting health.
    • Enthusiastic about collaborative work and has a keen interest in being part of a multidisciplinary team.

    Incentive - At 36 months guaranteed lump sum payment of $30K plus a $10 base salary adjustment.

    This is a normal M-F schedule but there may be very occasional consultative calls on the weekends, but they expect that to be light to non-existent.

    There is a $2K allowance for continuing ed and recertification.
