We are currently seeking a Director of Social Work to join our esteemed interdisciplinary team, contributing significantly to the emotional well-being of residents and their families.Role Summary:Under the supervision of the Administrator, the Director of Social Services is responsible for the planning, organizing, implementing, evaluating and directing of the Social Services program in accordance with current, federal, state regulations, industry best practices and uphold NASW standards. Leads Social Work and Case Management staff in providing medially relates social services that assist the residents to attain or maintain the highest practicable physical, mental and psychosocial well-being, navigating the health care system. Responsible for coordination of discharge planning and case management.Key Responsibilities:
- Guides facility staff in matters of resident advocacy, protection and promotion of residents’ rights.
- Demonstrates management skills sufficient to oversee the organization of department staff, to allocate resources and to supervise interdisciplinary cooperation in the planning and implementation of individual resident plans of care.
- Responsible for implementation of Social Work & Case Management initiatives and training of policies and procedures that govern social services. Demonstrates working knowledge and ability to interpret and implement facility policies and procedures to staff. Demonstrates working knowledge of laws and regulations that influence provision of care and services in nursing facilities
- Prepares for the long-term care survey. Interacts with State surveyors.
- Demonstrates assessment skills sufficient to evaluate residents’ behavior, to collect data and to evaluate psychosocial needs, risk factors for psychosocial deterioration and residents’ responses to interventions. Performs administrative requirements such as completing necessary forms and reports, including MDS and care plans.
- Able to implement social service interventions that achieve treatment goals, address resident needs, link social supports, physical care and physical environment to enhance quality of life.
Qualifications:- Current, unencumbered Social Worker License to practice in the State being employed
- Bachelor’s Degree in Social Work. Master’s Degree, preferred
- Long Term Care / Subacute Experience is required, demonstrating a profound understanding of the healthcare environment.
- One year of management experience preferred
