Care Coordination is a process that facilitates connecting children and their families with appropriate services and resources in a coordinated effort to achieve good health. The care coordinator is part of the pediatric medical care team and works side by side with physician and nurses in the hospital and clinic setting. The care coordinator is a direct point of contact for the patient and family in navigating medical services through St. Lukes Childrens Hospital, as well as community services and school entities.

The Care Coordinator works with providers, care team, patients, and families to implement a plan of care for assigned patients.
