Full-time Onsite Position

We are currently seeking a skilled and experienced Nurse Practitioner to join our dynamic team. As a Nurse Practitioner with our organization, you will play a crucial role in providing high-quality care to individuals with complex health needs.

Requirements:

• Master's degree or higher
• Valid Nurse Practitioner (NP) license in the state of Ohio (OH)
• 3 years of experience in Value Based Care, Primary Care, and Home Visits
• Experience with HCC Coding preferred
• Proficiency in Microsoft Office Suite
• Intermediate computer skills

Responsibilities:

• Conduct comprehensive clinical assessments, including history-taking and physical examinations
• Coordinate care among medical and behavioral health providers, making appropriate referrals
• Provide clinical support and guidance to team members
• Perform medication reconciliation and communicate with individuals' care providers
• Conduct home visits as needed to ensure continuity of care

Compensation:

• $125,000-135,000 Salary base
• 10% Bonus Potential
• Incentive - At 36 months guaranteed lump sum payment of $30K plus a $10 base salary adjustment.
