  •  The primary purpose of your job position is to assist residents of the Assisted Living Facility in all aspects of daily living, including the administration of medications. The administration of medications shall be in accordance with established nursing standards, the policies, procedures, and practices of this facility, and the requirements of this state.

    Delegation of Authority

    As an Assisted Living Medication Aide, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

     Job Functions

    Major Duties and Responsibilities

     Administrative Functions

          ·         Follow the policies and procedures of the facility governing the administering of medications to residents.

    ·         Note and report errors in the administration of medications.

    ·         Assist in developing and implementing procedures or programs of the facility that seek to determine what medication errors are taking place, when, where, and why they occur, and how they can be prevented.

    ·         Perform administrative requirements, such as completing necessary forms, charts, reports, etc., and submit these as may be required.

    ·         Maintain a friendly, productive, working relationship with the resident, the resident’s responsible party, and other necessary individuals and/or agencies. 

    ·         Report any complaints or grievances made by residents to the Resident Services Coordinator and/or the Administrator.

    ·         Participate and cooperate with any surveys (inspections) made by authorized government agencies.

    ·         Participate in the development and implementation of checklists and schedules of work assignments, and follow checklists and schedules as developed and modified.

    ·         Communicate pertinent resident information to other assisted living staff, and to other departments, ensuring the continuity of care and services to the resident.

     

     

    Committee Functions

     

    ·         Participate in facility committees as directed.

    ·         Participate in the comprehensive assessment of the resident and the formulation of the resident service plan as directed.

     Drug Administration Functions

          ·         Accurately and safely prepare, administer, and document the oral and/or topical medications that are commonly used in this facility and that may be ordered for                         resident use by the attending physician or the Medical Director.

    ·         Verify that any medications brought into the facility by a newly admitted resident are examined and identified by the attending physician or the facility pharmacy or   pharmacist.

    ·         Follow the labeling policies and practices of the facility.

    ·         Ensure that all medications administered are properly labeled.

    ·         Assist with medication charting as directed by the Nurse Supervisor/Charge Nurse.

    ·         Follow facility procedures in regard to charting of medications.

    ·         Follow facility policies and procedures regarding the destroying of medications; assist in carrying out these rules under the direction of the facility pharmacist or Director of Nursing Services, including the documentation requirements.

    ·         Assist in documenting and removing medications that are discontinued by the attending physician.

    ·         Follow facility procedures governing drug receipt errors, including notifying the delivery source and the Director of Nursing Services.

    ·         Ensure that medication is returned and reordered except emergency drugs.

    ·         Ensure that documentation of the drug receipt error includes a written error report, signed by an observer/witness and you.

    ·         Follow facility procedures in holding medications for residents who are transferred from the facility or when a medication is discontinued by the attending physician.

    ·         Accurately measure, record, and report the vital signs of residents.

    ·         Observe and verify that medication is ingested or applied as directed. Document any instance where prescribed medication is not administered, including reason(s) for refusal.

    ·         Promptly notify the Charge Nurse, Nurse Supervisor, or Director of Nursing Services of any errors or reactions to medications by residents.

    ·         Follow the medication card practices of the facility.

    ·         Ensure that no medication is administered without a written, signed, dated order from an attending licensed physician.

    ·         Follow the facility procedure for recording the order in the resident's clinical record, including the type, route, dosage, frequency and strength of each medication.

    ·         Verify the identity of the resident before administering the medication treatment.

    ·         Follow the facility's procedures allowing residents to self-administer their medications.

    ·         Assist the Nurse Supervisor/Charge Nurse in monitoring and documenting all aspects of such self-administering.

    ·         Promptly notify the Nurse Supervisor/Charge Nurse of any failure of a resident to properly take any "self-administered" medications.

    ·         Follow established guidelines concerning the storage of drugs and biologicals.

    ·         Be familiar with the meaning and usage of common medical abbreviations, symbols, and terms relative to the administration of medications as used by the facility, pharmacists, and physicians.

    Resident Service Functions

    The following services should be provided in accordance with the resident’s Service Plan.  These services should be provided in a manner that encourages and maintains the resident’s independence to the greatest extent possible. 

         Provide assistance as needed with:

    ·         Dressing and undressing

    ·         Bathing

    ·         Toileting

    ·         Personal hygiene, shaving, dental hygiene, nail care, hair care, foot care, etc.

    ·         Mobility devices (cane, walker, wheelchair, crutches) including monitoring of safe transfer and ambulation techniques.

    ·         Care of personal adaptive devices such as eyeglasses, contact lenses, hearing aids.

    ·         Dining

    ·         Telephone

    ·         Bed making and bed changes.

    ·         Trash disposal

    ·         Laundry

    ·         Activities (independent and group)

    ·         Transportation and business affairs

    ·         Appointments away from facility (coordinate with NF Charge Nurse and Transportation Aide)

    ·         Services by providers from outside agencies, provided within the facility (home health, etc.)

    Housekeeping Functions

    ·         Performs daily housekeeping tasks as assigned

    ·         Clean floors, to include sweeping, dusting, damp/wet mopping, disinfecting, etc. (NOTE: Assure that appropriate caution/safety signs are properly set up prior to performing such duties.)

    ·         Clean carpets, to include vacuuming, deodorizing, and disinfecting.

    ·         Clean windows and mirrors as needed

    ·         Clean resident’s bathrooms per schedule

    ·         Clean public restroom daily

    ·         Clean laundry area as needed

    ·         Clean walls and ceilings by washing, wiping, dusting, spot cleaning, disinfecting, deodorizing, etc

    ·         Discard waste/trash into proper containers and reline trash receptacle with plastic liner.

    ·         Clean vacant rooms as assigned.

    ·         Assure that work/assignment areas are clean and that equipment, tools, supplies, etc., are properly stored at all times, as well as before leaving such areas for breaks, meal times, and end of the work day.

    ·         Discard infectious wastes into appropriate containers

    ·         Maintain supplies in resident’s room (toilet tissue, facial tissue, personal supplies, etc.)

    ·         Maintain supplies in common areas and public restroom

    ·         Clean up spills and stains as necessary

    ·         Maintain communication with Housekeeping Supervisor as to needed supplies and equipment.

    ·         Notify Housekeeping Supervisor when major housekeeping functions are needed (waxing and buffing floor surfaces, shampooing carpet, etc.)

    ·         Other housekeeping tasks as directed.

     

    Activity Functions

     

    ·         Assist Activity Department in development of activities, independent and group, that conform with the residents’ desires and preferences.

    ·         Participate, as directed, in providing and assisting with activities for the residents.

    ·         Notify Activity Director when activity supplies are needed.

    ·         Notify Activity Director of requests by the residents for activities or activity supplies

    ·         Other Activity functions as directed.

     Dining Functions

          ·         Assist Dietary staff in serving meals

    ·         Assist residents with meals as needed (cutting meats, opening packets, pouring drinks, etc.)

    ·         Clear tables following meals, gathering serving ware and delivering to dishwashing facility for cleaning.

    ·         Wiping tables and replacing table cloths

    ·         Properly storing serving ware when received from Dietary Department

    ·         Maintaining the kitchen area in a clean and sanitary manner

    ·         Maintaining an adequate supply of snacks and other food items in the kitchen area as directed.

    ·         Notify Dietary Department of special requests, guests who request meal, requests for room trays, etc., as soon as requests are made to ensure requests are honored.

    ·         Notify Dietary Supervisor of guest meals or other charges, in order to ensure proper billing

    ·         Other Dining functions as directed.

     Social Service Functions

          ·         Monitor residents for signs of depression, anxiety, withdrawal, or loss of cognitive ability.  Notify Social Services Director of concerns.

    ·         Encourage residents to maintain social contact (coming to activities, meals, church services, etc.)

    Laundry Functions

    ·         Provide clean linens in each resident’s room daily.

    ·         Gather dirty linens and personal laundry daily, and deliver to facility laundry

    ·         Assist Laundry personnel in delivering personal laundry to residents, and storing clean linen in the linen supply room.

    ·         Notify Social Services Director of any reported missing clothing, or clothing needs of the resident.

    ·         Assist residents, as needed, to launder personal items in public laundry.

     Environmental Service Functions

          ·         Ensure that all areas of the facility, including grounds are clean, safe, and in good repair. 

    ·         Ensure that all equipment, fixtures and furniture is clean and in good repair

    ·         Monitor resident rooms and common areas, and identify potential hazards.

    ·         Report maintenance issues promptly to Maintenance Director

    ·         Assist Maintenance Director in Preventative Maintenance Program as directed.

    Documentation and Reporting Functions

    ·         Maintain a working knowledge of each Resident’s Service Plan, including review for changes in condition or needs.
