SUMMARY
Assume 24-hour accountability for the management of resident care, across locations, within the AL. Lead a multidisciplinary team responsible for identified program outcomes focused on clinical care, memory care and patient satisfaction. Serve as liaison between Assisted Living and the entire campus. Responsible for the assessment, planning, implementation and evaluation of activities that directly affect resident care. Provide ideas for new clinical and support services as well as enhancing existing services and programs.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Plan-How to meet established standards and ensure staff is accountable to the standard
1. Confirm annual program outcomes with executive team and evaluate performance against these outcomes on a regular basis.
2. Clarify accountabilities for resident care and satisfaction across multidisciplinary lines.
3. Clarify rewards and consequences linked to accountabilities
4. Maintain flexibility in self-scheduling in order to assess 24-hour, 365-days nursing care delivery.
5. Maintain consistent standards of care and accountability across locations.
6. Assure staffing patterns meet resident needs based on acuity.
7. Meet at a minimum of monthly with all shifts on their shift.
Coach, Teach and provide Leadership to program staff.
1. Facilitate meetings with the multidisciplinary team, as needed, to coordinate resident health services to assure seamless services to residents.
2. Collaborate with Administration to identify and implement staff development with educational and training initiatives including program orientation.
3. Participate in the hiring of all program staff.
4. Perform and coordinate staff evaluation (Annual and Probationary).
5. Monitor and report any employee performance or disciplinary issues for necessary action in conjunction with HR and executive team as necessary.
6. Oversee staff scheduling and fill in as need arises to ensure adequate patient coverage.
7. Give clear concise feedback to employees in a timely manner regarding unusual or critical events involving residents, staff, or families.
Regulatory Compliance
1. Complete investigations of incidents and accidents according to Corporate and State requirements.
2. Implement and enforce all corporate policies and procedures.
3. Maintain safety standards and institute safety precautions as appropriate.
4. Track and trend all Quality Indicators for use at Quality Assurance Performance Improvement Committee.
5. Evaluate and identify cause/effect, complete root cause analysis as necessary.
6. Implement Plan of Correction and implement strategies/approaches to prevent reoccurrence.
7. Update P/P at least annually and with changes in SOP.
8. Perform ISP at a minimum of every 6 months and with a change in the resident’s condition.
9. Develop collaborative relationships within the community to ensure the ability to meet the residents’ needs throughout the campus continuum.
Clinical Expertise
1. Make resident visits to monitor, assess, and implement appropriate interventions to assure resident care needs are being met by the interdisciplinary team involved with the program.
2. Maintain current CPR certification.
3. Review documentation to ensure adherence to policies and procedures and State and Federal guidelines.
4. Lead the development of new clinical interventions and services to improve resident care, outcomes and satisfaction.
5. Perform chart audits, including monitoring of physician and nursing orders to ensure they are carried out correctly and documented properly.
6. Maintain Nursing Licensure or CBRF certification in all modules including medication pass.
7. Review all clinical documentation to ensure adherence to current standard of practice, P/P, and state & federal regulations.
Communication
1. Facilitate communication with/between departments, administration, physicians to monitor visitation schedules, residents/families.
2. Keep administration informed of staffing needs, personnel concerns, environmental/equipment needs, unusual or critical events involving residents, staff, or families.
3. Establish environment in which staff, resident and visitor relations are fostered.
4. Identify concerns, formulate solutions, and coordinate facility resources to address issues pertaining to the program.
Budget/Finance
- Staffing levels are reviewed daily to assure budgetary compliance.
- Monitor time cards for accuracy.
- Monitor and control expenses against approved plan.
- Review budget variances with Administration.
SUPERVISORY RESPONSIBILITIES
Directly supervises up to 50 employees in the Care Delivery Department. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, directing work and staffing and schedules; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. Is responsible 24 hours a day.

OTHER QUALIFICATIONS
Above average assessment skills in geriatric patients and supervisory experience is preferred.
EDUCATION and/or EXPERIENCE
Bachelor's degree or Associate Degree in Healthcare related field. Current RN and/or CBRF licensed administrator.
LANGUAGE SKILLS
Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to write speeches and articles using original or innovative techniques or style. Ability to make effective and persuasive speeches and presentations on controversial or complex topics to top management, public groups, and/or boards of directors.
MATHEMATICAL SKILLS
Ability to apply advanced mathematical concepts such as exponents, logarithms, quadratic equations, and permutations. Ability to apply mathematical operations to such tasks as frequency distribution, determination of test reliability and validity, analysis of variance, correlation techniques, sampling theory, and factor analysis.
REASONING ABILITY
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
