    The Family Service Department’s primary role is to develop strong relationships with families in order to provide service coordination, support, and training to assist families in meeting the needs of their children. They serve as the primary point of contact for family members, ensuring they maintain an active line of communication.

    Job Description

    The Adult Services Coordinator at Grant a Gift Autism Foundation (GGAF) – Ackerman Center plays a vital role in providing comprehensive support and resources to adults with autism and other neurodevelopmental disabilities. This position involves coordinating care, facilitating communication between clients and healthcare providers, and ensuring access to necessary services and programs. The Adult Services Coordinator will work closely with clients and their families to develop and implement personalized care plans, advocate for their needs, and provide ongoing support and guidance.

    Duties and Responsibilities

    · Provide high-quality, patient-centered case management support to adults with autism and other neurodevelopmental disabilities.

    · Establish and maintain effective communication with clients and their families throughout the care coordination process.

    · Deliver intensive case management for Vocational Program clients to support as they work towards employment.

    · Act as a liaison between clients and Vocational Program sites to ensure that the needs of both parties are understood and met effectively

    · Support clients and families with transition planning as they grow into adulthood.

    · Organize and facilitate social and educational groups and opportunities for the clients

    · Document client needs and care coordination activities accurately and comprehensively within an Electronic Medical Record system.

    · Ensure compliance with all relevant privacy and confidentiality regulations.

    · Stay updated on new developments in healthcare and disability services through ongoing training and education.

    · Follow up on all referrals to ensure continuity of care and address client/family needs.

    · Foster relationships and partnerships with community agencies to facilitate referrals and access to resources.

    · Participate in local resource fairs and other community events to promote awareness and access to services.

    · Assist with the coordination of program orientations, training sessions, and support groups for clients and their families.

    · Support clinical team during relevant clinic sessions and group activities.

    · Provide assistance to Spanish-speaking clients in navigating the healthcare system and addressing barriers to care.

    Educational Requirements

    · Bachelor's degree or licensure in social work, psychology, or similar field (preferred)

    Qualifications

    · QMHA or ability to obtain QMHA

    · Compassionate and patient-centered approach to care coordination.

    · At least 1 year of experience working with low income and/or vulnerable populations – does not have to be autism/neurodevelopmental conditions related.

    · Ability to discuss sensitive issues with families using compassion and empathy.

    · Experience and working knowledge of healthcare regulations, and federal support systems (e.g., Medicaid, Medicare, SSI, and other government and commercial insurances).

    · Experience working within a non-profit, clinical training center (Preferred)

    · Strong working knowledge of Microsoft Office, especially Excel, Word and Outlook.

    · Excellent written and oral communications skills.

    · Creative, professional, and resourceful with the ability to work independently and a part of a multidisciplinary team.

    · Ability to take initiative and complete multiple tasks and projects on time.

    · Previous experience working within an Electronic Medical Record (EMR).

    · Detail orientated and strong organizational skills.

    · Bilingual in English and Spanish a plus.

    Physical Requirement

    · May include standing, sitting, and/or walking for extended periods

    · May include working on a special schedule (i.e., evenings and weekends for outreach)

    · May include working with challenging patients and clients

    · May include lift up to 50 pounds

    · May require ability to transport from one site to another
