The ED Discharge Care Coordinator provides referral coordination post-discharge in accordance with established policies and procedures of MidAmerica Division and participating hospitals. In collaboration with the hospital’s medical staff, patients, patient’s family and other members of the interdisciplinary health care team, the ED Discharge Care Coordinator is a central point of contact for coordinating and facilitating the referral process. The Coordinator will also collaborate with physician offices receiving referrals to ensure continuity of care.
