Ambulatory Services Director directs the staff and programs of the ambulatory services department. Oversees, administers, and coordinates all activities and operations of the ambulatory services for the health care facility, including outpatient clinics, outpatient surgery, and urgent care centers. Being an Ambulatory Services Director develops and introduces approved standards and guidelines for ambulatory services and programs. Maintains working relationships with community agencies. Additionally, Ambulatory Services Director ensures non-emergency care clinics operate within budget and in accordance with performance standards. Requires a bachelor's degree. Typically reports to top management. The Ambulatory Services Director typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. Capable of resolving escalated issues arising from operations and requiring coordination with other departments.     The Social Worker is responsible for the counseling of assigned clients as well as their family members. The Social Worker acts as a resource person to other program staff and to the facility in the area of counseling, family dynamics/interactions. The Staff Social Worker is under the direct supervision of the Administrator.

    DUTIES PERFORMED:

      • Conducts psychosocial history to determine pre-injury issues which need to be addressed.
      • Establishes and carries out individualized treatment plan upon completion of psychosocial history if applicable.
      • Provides counseling to clients, families and/or significant others.
      • Develops and implements family and children support groups as needed.
      • Schedules, arranges and moderates the program for family education group.
      • Participates in team rounds and family conferences as scheduled through case manger.
      • Maintains records and prepare reports according to facility procedures.
      • Participates in and documents internal and external professional educational activities to enhance personal and professional growth.
      • Performs public relations task as directed by supervisor.
      • Coordinates client's schedule with physical therapy, occupational therapy, therapeutic recreation, nursing service, speech and language pathology, behavioral specialist and other disciplines as needed.
      • Is an integral part of the interdisciplinary team.
      • Assists in patient discharge planning.
      • Conducts post discharge follow-up as needed.
      • Any other tasks as assigned by the immediate supervisor.

    SPECIFICATIONS

      • Formal Education/Formal Training: Must hold a Degree in Social Work and applicable license/certification.
      • Job Related Experience: Minimum of two years experience in a health care setting, preferably and Inpatient Rehab Facility (IRF)
