The Susan J Rheem Adult Day Center is in search of a part time Care Manager to join our team. This dynamic person will connect Susan J. Rheem Adult Day Center participants and their families with resources in the community to enhance their overall well-being. Our care manager is responsible for completing assessments, establishing and maintaining community connections for health and human service resources, maintaining detailed records and documentation of care management services provided, and is a part of our care planning team.

This is a part time position with Monday – Friday hours. No nights, weekends and most holidays off.

The ideal candidate will possess the following skills and requirements;

· Must have experience in an administrative capacity in a human service or health related organization. Preferably 1 years of current case management experience.

· Bachelors degree in health and human services or social work desired but will accept health and human services work experience in lieu of education.

· Strong organizational and communication skills.

· Compassionate, outgoing and positive attitude.

· Must be 21 years of age or older.

· Ability to obtain a Level One Fingerprint Clearance Card

· Must pass urine drug screen.
