Compensation: $150 to $200 per visit mileage reimbursement

Home Health Admission & OASIS RN Responsibilities:

·         Perform Admission / "Start of Care" OASIS Visits throughout an area of the Metro

·         Evaluate and assess clients for home health services and create their plan of care

·         Recommend services from other disciplines that have not yet been ordered by the physician

·         Collaborate with clients, families, physicians, and other members of the home care team

Home Health Admission & OASIS RN Qualifications:

Licensed as a Registered Nurse in the state of Minnesota

·         Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License

·         Prior Home Health Nursing experience

·         Proficiency with Medicare & OASIS documentation

·         Demonstrated ability to work autonomously

·         Excellent at planning, organizing, and communicating
