Long term Care Admissions Coordinator Opportunity!

It is the duty of the Admissions Coordinator to manage the facility inquiry process & admission process and provides customers with facility related information via facility tours, personal visits/assessments, conversations and follow-up.

Duties:

• monitoring open Referrals
• Communicate w/ Executive Director and/or DON to assure referrals are relied to in a timely manner
• Bed Management
• Facility Tours
• Welcome patient and/or family to facility- Answer questions, introductions to staff and facility
• Follow up with new resident and family members - follow up with patient daily for the first week and family members also. Any concerns need to be addressed with SSD and the Executive Director
• Participate and assist with facility events
• Assist w/ setting up transport to our facility once accepted (work with DBD on this)
• Ensure Nursing staff are alerted to new admissions coming
• Assist w/ follow up calls on those patients that are discharged to home.
• Assist w/ some marketing duties/events- set bingos
• Assist w/ facility activities as needed and as directed by the Executive Director
• Other duties as assigned by Management
