Addiction Mental Health Assessor jobs in Leominster, MA

Addiction Mental Health Assessor conducts initial and ongoing assessments of patients in Alcohol and Drug (AoD) addiction programs to determine mental health needs and establish care requirements. Provides initial treatment and refers clients to appropriate agencies/providers as needed. Being an Addiction Mental Health Assessor works with patients and families to develop appropriate chemical dependency service plans. Complies with standards in the area of evaluation, assessment, treatment, and service delivery to clients. Additionally, Addiction Mental Health Assessor requires certification as a Licensed Chemical Dependency Counselor (LCDC) or similar. Typically requires a bachelor's degree. May require a state license to practice. Typically reports to a supervisor or manager. Addiction Mental Health Assessor's years of experience requirement may be unspecified.     Responsibilities include:

    • Establishes positive, trusting rapport with client
    • Responsible for creative treatment planning & interventions to assist clients in achieving & exceeding treatment goals
    • Regularly meets with clients for ongoing therapy treatment
    • Complies with National Standards of Care set forth by Massachusetts State Licensing Board or any state where therapist is licensed
    • Complies with federal, state & HIPAA regulations, as it relates to medical records and confidentiality
    • Maintains Electronic Medical Record & completes all required documentation according to legal standards
    • Submits all documentation within 24 hours including documentation of any / all collateral contact
    • Benefit Eligible Clinicians will maintain a caseload of a minimum 25 billable client sessions / collateral contact hours per week
    • Participate in Integrated Care Model by coordinating services with collateral contact providers. Including but not limited to; family members, prescribers, school personnel, Intensive Care Coordinators, Therapeutic Mentors, Case Managers, etc. whenever appropriate
    • Providing referrals for additional care or services when required
    • Attend & actively participate in monthly 1:1 meetings with Clinical Director
    • Attend & actively participate in monthly group supervisions & staff meetings
    • Respond to communications and requests from Clinical Director in a reasonable & professional time frame
    • Completing all requirements to maintain Licensure
    • Review & follow practice & company’s policies & procedures as detailed in provider manual documents
    • Any additional responsibilities as assigned by Clinic Director


    Required Qualifications and Skills:



    • Candidates should have clinical licensure (LMHC, LMFT, LPCC, LICSW, LP etc.)
    • Required experience with completing DAs, treatment plans and clinical case notes
    • Effective written and verbal communication skills
    • Ability to demonstrate and model stable, appropriate boundaries with clients
    • Ability to complete and submit documentation of services and other documents in a timely manner
    • Comfort and familiarity working with a diverse client base
    • Proficient in the use of Office 365 and Electronic Health Record systems (Valant experience a plus!)
