    The Registered Nurse (Clinical Assessor) requires advanced knowledge and understanding of the services and benefits available for Veterans and caregivers from both federal, state, and private sector resources. The RN participates in face-to-face or virtual contact with the caregiver and Veteran to assess function and the need for personal care services. As a member of the interdisciplinary team, they will participate in development and application of directed clinical treatment interventions.

    Duties include, but are not limited to:

    • Implements the Caregiver Support Program specific to his/her area of assignment and ensures compliance and collaboration with local facility and the Veterans Administration Central Office Caregiver Support Program initiatives.
    • Conduct interim and annual home visits to Family Caregivers approved in the program.
    • Works directly with the facility Caregiver Support Coordinator to implement caregiver support services and benefits.
    • Participates in direct caregiver and Veteran contacts to identify and assess caregiver stress burden and the development and application of directed clinical treatment interventions.
    • Uses clinical knowledge to raise awareness of the value of caregiver issues and improve the quality and access to caregiver services.
    • Serves as a consultant to others in incorporating caregiver issues at the facility, VISN and national levels.
    • Provides referral to VA medical and mental health treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs.
    • Provides referrals or consults, or in some cases directs the admission of a Veteran to other VA programs including the Community Residential Care Program, Respite Program, Community Health Care Coordination Program, Hospice, Home Based Primary Care, Community Nursing Home Program and others.
    • Applies knowledge, including current trends and relevant theory related to operational efficiency, evidenced based practice & optimization of care delivery systems, and skill in the educational planning process including objective development, determining program content, faculty selection, learning activity implementation, teaching techniques, audiovisual use and program evaluations.
    • Conducts caregiver and family clinical interventions to facilitate the identification of caregiver stress, psychiatric illness, emotional distress and social needs as treatment issues.
    • Coordinates and integrates findings from needs assessments, inter-rater reliability assessments and program evaluations and incorporates into learning activities.
    • Assists in coordinating events for the annual National Family Caregivers Month to include announcements and advertising, mobilization of participants, educational sessions, caregiver recognition events and caregiver support programs.
    • Safely and effectively uses equipment required for patient care. Reports equipment in need of repair.
    • Recognizes subtle abnormal signs and symptoms and/or changes in patients' condition and responds appropriately.
    • Candidate must possess a current valid state drivers license.

    Basic Requirements:
    • English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
    • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD).
    • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)
