Addiction Mental Health Assessor conducts initial and ongoing assessments of patients in Alcohol and Drug (AoD) addiction programs to determine mental health needs and establish care requirements. Provides initial treatment and refers clients to appropriate agencies/providers as needed. Being an Addiction Mental Health Assessor works with patients and families to develop appropriate chemical dependency service plans. Complies with standards in the area of evaluation, assessment, treatment, and service delivery to clients. Additionally, Addiction Mental Health Assessor requires certification as a Licensed Chemical Dependency Counselor (LCDC) or similar. Typically requires a bachelor's degree. May require a state license to practice. Typically reports to a supervisor or manager. Addiction Mental Health Assessor's years of experience requirement may be unspecified.     Purpose of Position:

    Provides mental health assessment, consultation, and intervention services for the OBRA, Behavioral Health (BH), and Nursing Facility Monitoring (NFM) programs. Per OBRA guidelines, completes comprehensive mental health assessments for individuals diagnosed as having a serious mental illness and/or intellectual/developmental disability who are seeking admission to or are residing in nursing facilities. Per Integrated Services of Kalamazoo (ISK) guidelines, provides direct supports coordination and/or case management mental health services for the BH and NFM programs to mentally ill individuals living in the community (age 55 or over) or in a nursing facility (all ages) who are at risk of psychiatric hospitalization without on-going support and intervention.

    Essential Functions:

    OBRA Duties:
    • In compliance with OBRA guidelines, completes comprehensive mental health assessments for individuals diagnosed as having a serious mental illness and/or intellectual/developmental disability who are seeking admission to or are residing in nursing facilities. Makes written recommendations that include the following: diagnostic impressions, level of care needs, identification of least restrictive placement options, and the appropriateness/efficacy of mental health services in the nursing facility setting.
    • Provides education about OBRA process and guidelines to referral sources, nursing facilities, and hospitals as requested.
    • Provides back-up OBRA Coordinator duties when needed.
    BH and NFM Duties:
    • Carries a caseload for the BH and/or NFM programs. At the direction of the Clinical Manager, offers community based BH services to consumers referred by ISK and offers nursing facility based NFM services to those identified by the OBRA assessment as needing mental health intervention.
    • Case manages consumer’s overall delivery of service as stated in the Individual Plan of Service to ensure the comprehensive meeting of the consumer’s total unmet needs.
    • Collaborates with the appropriate medical, psychiatric in-patient, and ISK staff to facilitate the consumer’s discharge and safe return to the community from psychiatric hospital stays.
    • Provides consultation and education to family members of consumers to facilitate their understanding of the primary consumer’s needs and assist them in improving or preserving their involvement and relationship with the consumer.
    • Provides case consultation, staff education, training and support to nursing facility staff or AFC staff and other service providers to increase their awareness of the consumer’s physical and mental health needs related to planned services.
    General Duties:
    • Completes all documentation in a timely manner as required by OBRA, MDHHS and ISK guidelines.
    • Interacts with consumers served, legal representatives, natural supports, nursing facility staff, referral agents, colleagues, funder sources, and other community agents in a positive and professional manner.
    • Participates in all required team and agency meetings, in-services, and trainings.
    • Maintains professional Continuing Education requirements and State of Michigan licensure.
    • Complies with all federal, state, licensing, agency and funder requirements.
    • Other duties as assigned.
    Education/Experience:
    • The position requires a minimum of a Bachelor’s degree in a human service field plus current state certification or licensure in one of the following Michigan Licensures: RN, LBSW, LMSW, LPC, LP.
    • A minimum of a bachelor’s degree in a human services field and one year of experience with people with mental illness.
    • Preference will be given to persons with a combination of mental health experience and working with the elderly for at least two years.
